NACOSA is a networking organisation that brings communities together to tackle HIV and its impact on vulnerable people. We have a 19 year track record of working with donors, government and communities to strengthen community systems and turn the tide on HIV, AIDS and TB.
We do this through mobilizing our network of over 1,900 organisations, promoting dialogue between government and affected people, building capacity with accredited training, mentoring and technical assistance and channeling resources to support service delivery on the ground, particularly for children and youth, key populations and women and girls.

 

 

 

Job Title: Health Connectors
Location: Newcastle, KwaZulu-Natal, South Africa

Job Summary:
The Health Connector ensures proper linkage to care promotes emotional wellness to communities and individuals by providing quality HTS services to AGYW in order to become an integral part of the broader comprehensive care and treatment in a non-clinical site that links the community to care.

Duties and Responsibilities:

•Ensures that proper linkage to care of all clients is done according to DoH guideline
•Ensure that all clients who tested HIV positive are linked into care and keep record of how many are initiated on ART
•Provides HIV Testing Services to clients in line with DoH policies and guidelines
•Conduct tracing of patients who have missed their appointments or defaulted from care
•Keep an update record of number of clients offered HTS, tested, tested HIV positive, linked to care, etc.,
•Attend facility meetings as required to discuss HTS/TB related issues
•Educate clients on HIV, AIDS and STI’s and their implications
•Provides unbiased information on HIV testing to his/her clients & explains the testing process in simple way.
•Keep all the records, data about a patient confidential safe.
•Maintains a list of certifications of Rapid HIV Test.
•Conducts a report of risk assessment in which report he explains the implications of risk behaviours to clients.
•Prepares a medical follow up for critical cases that have high risk of HIV infection.
•Submits a monthly statistical report as required by the program supervisor to look after every client properly on time.

Education, skills and experience Required:
•Matric / Grade 12 / NQF4
•2 – 3 year’s relevant experience as a HAST Coordinator working in a HAST programme or health facility
•ATTIC Certification
•Successful completion of basic counselling course
•Completion of HIV/AIDS courses or certifications
•Practical experience working in the HIV/AIDS and Education field within the NGO and NPO sector working with AGYW
•Read, write, and speak in two official South African languages, of which one must be English.
•Ability to effectively present information and respond to questions
•Good computer and reporting skills
