{"id":42361,"date":"2018-08-24T11:40:41","date_gmt":"2018-08-24T09:40:41","guid":{"rendered":"https:\/\/joblistsouthafrica.com\/?p=42361"},"modified":"2018-08-24T12:21:34","modified_gmt":"2018-08-24T10:21:34","slug":"pact-new-job-position-share-apply","status":"publish","type":"post","link":"https:\/\/joblistsouthafrica.com\/pact-new-job-position-share-apply.html","title":{"rendered":"Pact New Job Position &#8211; Share &#038; Apply!"},"content":{"rendered":"<p>Pact, an international NGO based in Pretoria, seeks to employ a Health Referral &amp; Linkages Officer up to September 2020 to support its USAID Funded Government Capacity Building Program (GCBS).<\/p>\n<p><strong>Job Title: Health Referral &amp; Linkages Officer<\/strong><br \/>\n<strong>Job Location:<\/strong> Pretoria<\/p>\n<p>The Health Referral &amp; Linkages Officer (HRLO) will play a pivotal role in ensuring functional bi-directional referral systems between communities, clinical services, and social service providers. Proficiency in dealing with M&amp;E is preferred, as is expertise in referral systems.<\/p>\n<p><strong>Key Responsibilities<\/strong><\/p>\n<ul>\n<li>Provide case management and tracking for all children identified as HIV+ in selected provinces<\/li>\n<li>Ensure functional bi-directional referral systems between facilities and GCBS service points in selected provinces\/districts.<\/li>\n<li>Monitor success of the referral systems, and provide support to troubleshoot and problem-solve as needed;<\/li>\n<li>Provide monthly feedback on caseload of HIV+ children to DSD staff, NPO staff, project staff and health facility staff for programmatic decision making.<\/li>\n<li>Coordinate with HIV Testing and Counseling providers and conduct community outreach;<\/li>\n<li>Attend quarterly district level quality improvement meetings with relevant bidirectional referrals stakeholders as required;<\/li>\n<li>Coordinate with the GCBS Program and M&amp;E teams to ensure all bi-directional referrals data are accurate and produced weekly; use data for decision-making and ensure partners are using data to conduct needed follow-up for systematic follow up of cases of incomplete referrals;<\/li>\n<li>Report on referral systems and key PEPFAR performance indicators on a monthly basis;<\/li>\n<li>Conduct home visits to clients and case follow up as needed.<\/li>\n<\/ul>\n<p><strong>Minimum Education and Experience Requirements<\/strong><\/p>\n<ul>\n<li>Master\u2019s degree or higher, preferably in one of the Social Sciences;<\/li>\n<li>Designated Social Auxiliary Worker (SAW) or Child and Youth Care Worker (CYCW);<\/li>\n<li>At least three years\u2019 experience in a field position with an organization in public health or OVC programming; experience in developing and managing referral systems preferred;<\/li>\n<li>Experience with PEPFAR funded projects in South Africa;<\/li>\n<li>Excellent written and oral communication skills in English, including report writing;<\/li>\n<li>Able and willing to work flexible hours as needed and without constant or close supervision;<\/li>\n<li>Demonstrated leadership and team-building skills.<\/li>\n<\/ul>\n<p>Only applicants possessing the prescribed minimum qualifications and experience are invited to submit their electronic CVs and application letters with a subject line \u201c<strong>Health Referral &amp; Linkages Officer<\/strong>\u201d to\u00a0<strong>SAtechnicalapps@pactworld.org<\/strong><\/p>\n<p>Closing Date:\u00a0<strong>07 September<\/strong><strong>\u00a02018.<\/strong><\/p>\n<p>Pact reserves the right to verify all information provided by candidates including credit rating.
