Today, HPPSA employs over 917 people in 5 provinces (Gauteng, Limpopo, Mpumalanga and KwaZulu-Natal and Eastern Cape) and reaches over 500,000 people each year.<\/p>\n<p>Humana People to People in South Africa seeks to recruit a Community Services and Linkage Manager, based in Nelspruit on a 5-year contract.<\/p>\n<p><strong>Job Position:\u00a0Community Services and Linkages Manager\u00a0<\/strong><br \/>\n<strong>Project Name:\u00a0<\/strong>HPPSA OVC Program<br \/>\n<strong>Location:\u00a0<\/strong>Nelspruit, South Africa<br \/>\n<strong>Type:\u00a0<\/strong>Regular\/Full Time<br \/>\n<strong>Salary:\u00a0<\/strong>R35\u00a0000 &#8211; R45 00 PM (NO Benefits)<br \/>\n<strong>Reports to:\u00a0<\/strong>Chief of Party<\/p>\n<p><strong>Community Services &amp; Linkage Manager:<\/strong><\/p>\n<p>S\/he will be the lead technical expert for integration of community and clinical service delivery with a focus on improving results towards \u201c90-90-90\u201d.\u00a0 This includes strengthening linkages and coordination between social service and health workforces as well as strengthening bi-directional referral systems to improve both health\/HIV and social service delivery.\u00a0\u00a0 The Community Services &amp; Linkage Manager will strengthen implementation of comprehensive, HIV inclusive case management with a focus on improving pediatric HIV case identification; strengthening HIV prevention focus, linkage to HTS, care, and treatment; retention among both children and their caregivers; and violence prevention and response.<\/p>\n<p><strong>Key Performance Areas:\u00a0\u00a0<\/strong><\/p>\n<ul>\n<li>Strengthen bi-directional referral systems to improve both health\/HIV and social service delivery, particularly for OVCA and AGYW<\/li>\n<li>Improve implementation of comprehensive, HIV-inclusive case management with a focus on improving paediatric HIV case identification and response<\/li>\n<li>Strengthen HIV prevention focus, linkage to HTS, care, and treatment; retention among both children and their caregivers; and violence prevention and response among target populations<\/li>\n<li>Manage community profile and mapping referral systems of patients from the community to facility and vice versa<\/li>\n<li>Work closely with key stakeholders to create and manage demand for health services<\/li>\n<li>Develop systems to enhance linkages to care, trace defaulters, and ensure age-appropriate, targeted care in conjunction with community-based staff and health facilities<\/li>\n<li>Engage communities in target areas to improve the quality of care, retention in care, and adherence to treatment<\/li>\n<\/ul>\n<p><strong>Qualifications and Experience<\/strong><\/p>\n<ul>\n<li>A minimum of a Bachelor\u2019s degree in Social Work or Public Health with a focus on public health, social sciences, or related field<\/li>\n<li>Demonstrated role in strengthening coordination and linkages between clinical and community stakeholders, health and social service workforces; and operationalizing integrated service delivery models;<\/li>\n<li>A minimum of 5 years of progressively responsible experience in a technical advisory role for large-scale family and community-based programs;<\/li>\n<li>A minimum of 3 years of experience serving in a technical advisory role for a PEPFAR-funded activity, with a focus on improving the wellbeing of OVCA and HIV prevention among AGYW<\/li>\n<li>Demonstrated working knowledge of PEPFAR Technical Guidance for OVC and Prevention<\/li>\n<li>Solid knowledge of the public health landscape in South Africa<\/li>\n<li>Demonstrated written, presentation, communication and organizational skills in English<\/li>\n<\/ul>\n<p>For more about Humana People to People in South Africa, refer to\u00a0<a href=\"http:\/\/www.hpp-sa.org\/\">www.hpp-sa.org<\/a><\/p>\n<p>Applicants must email CV to\u00a0<strong><a href=\"mailto:applications@hpp-sa.org\">applications@hpp-sa.org<\/a><\/strong><\/p>\n<p><strong>Application Deadline:\u00a0<\/strong>18th Feb, 2019<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Humana People to People South Africa (HPP-SA) is a registered South African non-profit company and non-profit organization founded in 1995 with the mission of empowering the poor to lift themselves&#8230;..<\/p>\n","protected":false},"author":1350,"featured_media":0,"comment_status":"closed","ping_status":"open","sticky":false,"template":"","format":"standard","meta":{"om_disable_all_campaigns":false,"jetpack_post_was_ever_published":false,"_jetpack_newsletter_access":"","_jetpack_newsletter_tier_id":0,"footnotes":"","jetpack_publicize_message":"","jetpack_is_tweetstorm":false,"jetpack_publicize_feature_enabled":true,"jetpack_social_post_already_shared":true,"jetpack_social_options":{"image_generator_settings":{"template":"highway","enabled":false}}},"categories":[243],"tags":[],"jetpack_publicize_connections":[],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v21.5 -
