{"id":49186,"date":"2019-09-20T17:36:46","date_gmt":"2019-09-20T15:36:46","guid":{"rendered":"https:\/\/joblistsouthafrica.com\/?p=49186"},"modified":"2019-09-19T17:41:15","modified_gmt":"2019-09-19T15:41:15","slug":"hpca-latest-available-job-opportunity-apply-now","status":"publish","type":"post","link":"https:\/\/joblistsouthafrica.com\/hpca-latest-available-job-opportunity-apply-now.html","title":{"rendered":"HPCA Latest Available Job Opportunity &#8211; Apply Now!"},"content":{"rendered":"<p>The primary purpose of this position is to strengthen active referrals and linkages from community-based HIV testing points to HIV care and treatment and to improve retention in care through community-based adherence support interventions.<\/p>\n<p><strong>Job Title: <\/strong><strong>Testing, Linkage and Retention officers<\/strong><br \/>\n<strong>Location: <\/strong>South Africa<\/p>\n<p><strong>THE SUCCESSFUL CANDIDATE WILL:<\/strong><\/p>\n<ul>\n<li><strong>Implement Targeted Community HIV Testing Services<\/strong><\/li>\n<\/ul>\n<p>Increase and create demand for community-based HIV Testing Services through community mobilization &amp; campaigns and case finding by expanding access to traditional and non-traditional facility- and community-based HTS.<\/p>\n<p>Differentiate HTS for priority populations with age-appropriate and targeted approaches for children, AGYW and men and high-risk individuals; Leverage other critical entry points for HIV case finding and Liaise with Testing, Linkage and Retention Mentor to ensure supply of quality HIV test kits for use in communities; Liaise with Testing, Linkage &amp; Retention Mentor to ensure proficiency in HIV Testing; Support door to door index HIV testing in targeted high HIV prevalence and hard to reach areas. Coordinate targeted and integrated community outreach interventions such as Extended Program on Immunization mobile services to increase Early Infant Diagnosis and Paediatric HIV testing.<\/p>\n<ul>\n<li><strong>Implement Active Referral and Linkage to Care<\/strong><\/li>\n<\/ul>\n<p>Ensure that all clients tested HIV positive at community sites are referred and linked to facility for treatment initiation;<\/p>\n<p>Conduct follow up visits to facilities to ensure that patient are captured into Tier.Net; Implement &amp; monitor patient tracking logbooks and intervention to bring those lost to follow up into care.<\/p>\n<p>Work closely with WBPHCOTs &amp; CBOs and provide a list of patients to be physically traced;<\/p>\n<p>Set up networks and linkage with key stakeholder such as health facilities, Voluntary Medical Male Circumcision sites and Sexual Reproductive Health Services and Nutrition to ensure successful referrals;<\/p>\n<p>Improve bi-directional referral systems between community and facility-based services; Improve utilization of community-based health information systems to track community\/facility referral.<\/p>\n<ul>\n<li><strong>Implement Adherence Support and Retention in Care<\/strong><\/li>\n<\/ul>\n<p>Collaborate with WBPHCOTs and CBOs to trace Pre-ART patients and ART defaulters; Support community-based HIV services for new support groups and stable (Adherence Clubs) ART patients including cohorting, recruitment and facilitation; Work with NACS and SRH Technical Assistant to integrate Nutrition Assessment, Counselling, and Support (NACS) into Adherence Clubs.<\/p>\n<p>Provide Health Education to clients on HIV, ARVs, Adherence to ART, Viral Load monitoring and TB screening Timeously report any challenges related to the referral and linkage to care to foster early interventions.<\/p>\n<p><strong>QUALIFICATION AND EXPERIENCE<\/strong><strong>:<\/strong><\/p>\n<ul>\n<li>Matric \/ Grade 12 Certificate<\/li>\n<li>Proficiency in MS Office<\/li>\n<\/ul>\n<p><strong>1 to 3 Years\u2019 experience in:<\/strong><\/p>\n<ul>\n<li>HIV\/AIDS or Public Health field<\/li>\n<li>Tracing strategies<\/li>\n<li>Adherence Clubs Facilitation<\/li>\n<li>HIV Testing Services &amp; application testing of modalities<\/li>\n<li>Referral Systems and pathways, linkage and retention in care<\/li>\n<\/ul>\n<p><strong>COMPETENCY PROFILE:<\/strong><\/p>\n<ul>\n<li>Excellent interpersonal skills<\/li>\n<li>Planning &amp; Organising<\/li>\n<li>Customer centric focus<\/li>\n<li>Attention to detail<\/li>\n<li>Communication skills<\/li>\n<li>High level of integrity<\/li>\n<li>Strong administrative skills<\/li>\n<li>Assertiveness<\/li>\n<li>Problem Solving<\/li>\n<li>Compassion<\/li>\n<\/ul>\n<p><strong>To Apply<\/strong><\/p>\n<p>Please submit a detailed CV, covering letter and names of three contactable references including email contact to\u00a0<a href=\"mailto:hr@hpca.co.za\"><strong>hr@hpca.co.za.<\/strong>\u00a0<\/a><\/p>\n<p><strong>Application Deadline:<\/strong> \u00a0Friday, 27 September 2019<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The primary purpose of this position is to strengthen active referrals and linkages from community-based HIV testing points to HIV care and treatment and to improve retention in care through&#8230;..<\/p>\n","protected":false},"author":1353,"featured_media":0,"comment_status":"closed","ping_status":"open","sticky":false,"template":"","format":"standard","meta":{"om_disable_all_campaigns":false,"jetpack_post_was_ever_published":false,"_jetpack_newsletter_access":"","_jetpack_newsletter_tier_id":0,"footnotes":"","jetpack_publicize_message":"","jetpack_is_tweetstorm":false,"jetpack_publicize_feature_enabled":true,"jetpack_social_post_already_shared":true,"jetpack_social_options":{"image_generator_settings":{"template":"highway","enabled":false}}},"categories":[1],"tags":[],"jetpack_publicize_connections":[],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v21.5 - https:\/\/yoast.com\/wordpress\/plugins\/seo\/ -->\n<title>HPCA Latest Available Job Opportunity - Apply Now!
