This collaboration aims to improve access to palliative care for older people in South Africa through training, support and policy development.<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Job Title: Temp &#8211; Linkage Officers <\/strong><br \/>\n<strong>Location:\u00a0<\/strong>Free State<br \/>\n<strong>Duration: <\/strong>(3 Months Period)<\/p>\n<p><strong>Free State: Thabo Mafutsanyana District- Maluti-a-Phofung, Kentoana\/Phumelela, Kentoana<\/strong><strong>, Phumelela Local Municipality<\/strong><\/p>\n<p>The primary purpose of this position is to support HIV Lay Counsellor testing services and strengthen linkages from community-based HIV testing points to HIV care and treatment<\/p>\n<p><strong>THE SUCCESSFUL CANDIDATE WILL:<\/strong><\/p>\n<ul>\n<li><strong>Support Community HIV Testing Services as per HPCA annual Work plan<\/strong><\/li>\n<li>Collaborate with community stakeholders and DoH to create increased demand for community-based HTS<\/li>\n<li>Support Lay Counsellors with newly initiated clients<\/li>\n<li>Support Lay Counsellors to assist in clients undertaking the HIV self-test (HIVSS)<\/li>\n<li><strong>Implement Linkage to Care as per HPCA annual Work plan<\/strong><\/li>\n<li>Link all referred reactive clients tested to facility for treatment initiation<\/li>\n<li>Encourage clients who have a reactive screening on sight to have a Rapid Test immediately to confirm their status<\/li>\n<li>Follow up on clients who have accepted non-assisted HIVSS who used the test and provide support.<\/li>\n<li>Follow up on clients who have received a reactive result after self- testing and are not willing to undertake a Rapid Test<\/li>\n<li>Confirm the correctness of all contact information and complete clinical forms<\/li>\n<li>Provide enhanced adherence support to all clients who have been initiated on treatment<\/li>\n<li>Follow up and trace reactive clients who have not been linked to care by collaborating with WBPHCOTs and CBOs<\/li>\n<li>Maintain Linkage standards in line with the National HTS policy guideline and HPCA Linkage and Referral SOP.<\/li>\n<li><strong>Monitoring and Reporting as per HPCA annual Work plan<\/strong><\/li>\n<li>Advise the TLRO on newly initiated clients, to liaise with the data capturer to verify on Tier.Net<\/li>\n<li>Advise the TLRO if a client is initiated on treatment to enable updating of the tracking and tracing tool<\/li>\n<li>Advise the TLRO on outcomes of linkage activities for both rapid and HIVSS testing<\/li>\n<li>Report any challenges related to supporting testing or linkage to care to TLRM<\/li>\n<\/ul>\n<p><strong>QUALIFICATION AND EXPERIENCE (Minimum requirements)<\/strong><strong>:<\/strong><\/p>\n<ul>\n<li>Matric \/ Grade 12 Certificate or equivalent<\/li>\n<li>Accredited training certificates:<\/li>\n<li>Basic HIV information course, Intensive Counselling course and Adherence Counselling course<\/li>\n<li>Finger prick certificate<\/li>\n<li>Ability to communicate in English and one or more South African language(s) commonly spoken in the Sub-District<\/li>\n<li>Basic knowledge of HIV\/AIDS and TB &amp; Supporting people living with HIV<\/li>\n<li>Involvement in some aspect of community work<\/li>\n<li>Valid EB Driver\u2019s License (Advantageous)<\/li>\n<li>Clear criminal record<\/li>\n<\/ul>\n<p><strong>Minimum one to two years\u2019 experience in<\/strong>:<\/p>\n<ul>\n<li>HIV Counselling and adherence Counselling<\/li>\n<\/ul>\n<p><strong>KNOWLEDGE, SKILLS AND COMPETENCIES REQUIRED:<\/strong><\/p>\n<ul>\n<li>Technical Skill and expertise<\/li>\n<li>Good communication Skills<\/li>\n<li>Customer Service orientation<\/li>\n<li>Detail Orientation<\/li>\n<li>High level of integrity<\/li>\n<li>Team work<\/li>\n<li>Adaptability<\/li>\n<li>Compassion<\/li>\n<\/ul>\n<p><strong>To apply <\/strong><\/p>\n<p>Please submit a detailed CV, covering letter and names of three contactable references including email contact to<strong>\u00a0<\/strong><a href=\"mailto:hr@hpca.co.za\"><strong>hr@hpca.co.za.<\/strong>\u00a0<\/a><\/p>\n<p><strong>Application Deadline:<\/strong> Friday, 20<sup>th<\/sup> December 2019<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The Hospice Palliative Care Association of South Africa (HPCA) has a memorandum of understanding (MOU) with The Association for the Aged (TAFTA).
