The organisation empower families to create their own future \u2013 for the sake of our children and for the collective future of South Africa.<\/p>\n<p>Future Families provides care and support to families with members who are infected and affected by HIV and AIDS to ensure that children in need have a safe, nurturing environment to grow into independent, empowered adults capable of planning their own future.<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Job Title:\u00a0Linkage Officer<\/strong><br \/>\n<strong>Location:<\/strong> City of Johannesburg (Diepsloot &amp; Ivory Park) and\u00a0\u00a0Ekurhuleni North.<\/p>\n<p><strong>Requirements:<\/strong><\/p>\n<ul>\n<li>Qualified Nurse<\/li>\n<li>Minimum 2 years experience<\/li>\n<li>Driver&#8217;s License and own vehicle to be able to travel between offices<\/li>\n<li>Computer literacy (MS Office (Outlook, Word and Excel)<\/li>\n<li>Be able to work independently<\/li>\n<li>Sound Knowledge of HIV\/AIDS<\/li>\n<li>Deadline driven<\/li>\n<\/ul>\n<p><strong>Responsibilities:<\/strong><\/p>\n<ul>\n<li>Ensure implementation of health related programme activities at their allocated sites.<\/li>\n<li>Assist with the linking WBOT&#8217;S with OVC Partners for tracing LTFU and delivering ART to at risk beneficiaries<\/li>\n<li>Collaborate in the tracing and follow up of (children &lt;2) LTFU in PMTCT cascade<\/li>\n<li>Support bi-directional referrals of C\/ALHIV, with DSP&#8217;s referring new and existing C\/ALHIV, prioritising patients with suppressed viral loads.<\/li>\n<li>Well-developed interpersonal skills and excellent patterns of communication.To apply, submit your applications to\u00a0<strong><a href=\"mailto:marie@futurefamilies.co.za\">marie@futurefamilies.co.za<\/a><\/strong><\/li>\n<\/ul>\n<p><strong>Application Deadline:\u00a0<\/strong>Tuesday, 15 September, 2020<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Future Families is a nonprofit organisation established on 1st October 2010 which operates Tshwane South Africa.
