The package consists of interventions that focus on health and wellness (ranging from prevention, promotion, rehabilitation and palliation).<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Key Duties and Responsibilities:<\/strong><\/p>\n<p><strong>Physical Care<\/strong><\/p>\n<ul class=\"bullet-list\">\n<li>Assist the patient with personal hygiene.<\/li>\n<li>Wound care, how to dress a wound using appropriate material.<\/li>\n<li>Stoma therapy, catheter care, nail and foot care, tracheotomy care.<\/li>\n<li>Observations: temperature pulse and respiration.<\/li>\n<li>Be able to assess an emergency situation and provide basic life support and first aid in order to stabilize a patient.<\/li>\n<li>Assist patient with mobility, exercise limbs, take patient for walks, assist in and out of bed and to the toilet, have knowledge of motion exercises.<\/li>\n<li>Preparation of patient&#8217;s snacks and meals if necessary.<\/li>\n<li>Distribution of food parcels where necessary and if patient&#8217;s circumstances meet the required criteria.<\/li>\n<\/ul>\n<p><strong>\u00a0Care of Patient\u2019s Environment<\/strong><\/p>\n<ul class=\"bullet-list\">\n<li>Make patients bed, replacing with clean bed linen when necessary.<\/li>\n<li>Tidy and clean the patients\u2019 sleeping quarters, disposing of any soiled night clothes and bed linen and washing of these if necessary.<\/li>\n<li>Infection control.<\/li>\n<\/ul>\n<p><strong>\u00a0Health and Education<\/strong><\/p>\n<ul class=\"bullet-list\">\n<li>Educate patient and relatives about caring for the patient at home and resources available.<\/li>\n<li>Specialized care of catheters etc.<\/li>\n<li>Universal blood precautions.<\/li>\n<li>Nutrition: Identify food groups and educate patient\/family on planning suitable meals for their needs<\/li>\n<\/ul>\n<p><strong>\u00a0Referral Criteria<\/strong><\/p>\n<ul class=\"bullet-list\">\n<li>Refer patients to health facilities and service providers where necessary.<\/li>\n<li>Know the when, why and how of referrals.<\/li>\n<li>Reporting of deterioration of patient\u2019s condition to supervisor<\/li>\n<\/ul>\n<p><strong>\u00a0Administration &amp; General Duties<\/strong><\/p>\n<ul class=\"bullet-list\">\n<li>Establish a set of records and ensure that they are up to date.<\/li>\n<li>Complete the appropriate record forms accurately and promptly ensuring that these are given to the supervisor timeously.<\/li>\n<li>Follow all policies and procedures as required by the organization and ensure that all work targets are met.<\/li>\n<li>Follow daily work schedule as worked out by the supervisor.<\/li>\n<li>Participate in meetings as required as well as attending training courses and workshops related to the nature of the work done.<\/li>\n<\/ul>\n<p><strong>\u00a0Medication<\/strong><\/p>\n<ul class=\"bullet-list\">\n<li>Supervision\/administration of medication.<\/li>\n<li>Collection of prescribed medication (repeat) from the CHC<\/li>\n<li>Ensure as far as possible that the patient completes their treatment in the prescribed time given.<\/li>\n<li>Report any patient that defaults on treatment immediately to the staff of the CHC.<\/li>\n<li>Report any reaction to any medication to the OTL.<\/li>\n<\/ul>\n<p>Any other tasks as agreed with line manager.<\/p>\n<p><strong>Minimum<\/strong>\u00a0<strong>Qualifications,<\/strong>\u00a0<strong>Experience<\/strong><strong>\u00a0and Skills Required<\/strong><strong>\u00a0<\/strong><\/p>\n<ul class=\"bullet-list\">\n<li>Grade 10-12<\/li>\n<li>Homebased carer qualification\/certificate<\/li>\n<li>Experience working within communities<\/li>\n<li>10-day HIV Counselling Certificate (advantageous)<\/li>\n<li>TB\/HIV Adherence Counselling Certificate (advantageous)<\/li>\n<li>Must have a background knowledge of HAST &amp; NCD<\/li>\n<\/ul>\n<p>These positions will be based in Parow, Gugulethu, Kraaifontein, Straandfontein and Westlake.
